Agewell in Sheffield, in conjunction with NHS Sheffield’s Community Intermediate Care Scheme launched their ‘First Point of Contact’ Scheme on 14 February 2011.
Agewell became an agency for the NHS CICS Team (Community Intermediate Care Service) to refer clients on for third sector agencies such as benefits advice, Lunch Clubs, befriending, shopping etc.
Patients referred to Agewell from the CICS team have had a recent hospital episode often because of a fall, or have been referred to prevent hospital admission due to acute problems such as falls or uti. The aim is to enable these clients to remain independent in their own homes.
Agewell has created a data base of third sector agencies which is constantly growing and updated and staff can search for a range of services suitable to needs required.
